Volunteering Matching Submission Request Form
To complete a request for your volunteering activities to be matched complete the following fields which are mandatory except where otherwise noted:
Charity
Select the registered DGR or non-DGR charity you have volunteered for by entering the first three letters into the charity search box and then selecting your chosen organisation. Only charities which are registered on the Workplace Giving Platform are eligible for Volunteer matching. If your chosen charity is not available, you can request a new charity to be added to the platform by clicking on the “add a new charity” link.
Volunteering Activity Name
Enter the name that describes the volunteering you have completed.
Volunteering activity date range
Provide the start and end dates of your volunteering activity. You only need to specify the month and year using the drop down options on the form.
Volunteering hours completed
Enter the total number of hours you volunteered for this charity.
Additional information to send to this charity (optional)
If you would like to provide any additional information to your charity, you can enter this here. A maximum of 100 characters can be provided.
Individual or Team Donation
Please indicate whether the Volunteering relates to an Individual or Team activity.
Please don’t pass my details to this charity
Select this option if you want to remain anonymous to the charity and your contact details will not be provided to the charity.
Evidence File Date
Please enter the date which the evidence file was created or otherwise provided by the charity.
Evidence of volunteering
Upload a donation evidence receipt file which confirms the charity name, time period when you volunteered, and the number of hours that you volunteered. This could be a scanned image, photo or electronic copy letter from the charity.
Please note the Workplace Giving Platform accepts the following file formats: .doc, .docx, .jpg, .jpeg, .png or .pdf.
